Free Dialysis Fluid Log Printable (PDF)

Free dialysis printable PDF

A reusable weekly fluid log for recording what you drink, when you drink it, and your daily total between dialysis treatments.

For people on hemodialysis, extra fluid can build up between treatments and then has to be removed during dialysis. Your own fluid allowance is individualized, but keeping a simple record can make it easier to see where most of your intake is coming from and whether certain times of day are harder than others.

What this printable helps you track

  • Fluid intake in the morning, afternoon, and evening
  • Your daily total
  • Your care team’s fluid allowance, if one has been prescribed
  • Patterns that may be useful to discuss with your dialysis dietitian or nurse

What counts as fluid?

It is easy to think only about water, but kidney-care guidance also counts foods that are liquid at room temperature. Depending on the plan your care team gives you, examples can include coffee, tea, juice, soup, gelatin, ice cream, popsicles, gravy, and ice. Some foods also contribute water even though they are not liquid at room temperature.

You do not need to turn every meal into a math exercise. The goal of this sheet is to give you one place to record the fluids your team has asked you to count so you can see your day more clearly.

Why a daily pattern can be more useful than memory

Many people do well earlier in the day and then feel much thirstier in the evening, or notice that salty meals make the next several hours harder. A log can show those patterns without relying on memory at your next appointment. It can also make it easier to ask specific questions, such as whether changing cup size, timing, or food choices might help you stay within the plan your team gave you.

This printable does not set a fluid limit. Fluid needs vary based on urine output, dialysis schedule, heart function, medications, weather, and other factors. Write down the allowance your own care team gives you rather than copying someone else’s number.
