Weight, fluid, blood pressure, whether treatment ran full-length, and how you felt are five things your care team already watches closely — and they’re easier to see as a pattern when they’re recorded together instead of scattered across separate sheets. Use this printable if you’d rather fill in one row per treatment than juggle several logs; if you want to track any one of these in more depth, the individual logs are linked below.
Use it to keep track of
- Your weight before and after each treatment
- Fluid intake since your last treatment
- Blood pressure and pulse, pre- and post-treatment
- Whether treatment ran full-length, was shortened, or was missed
- Symptoms or how you felt during and after treatment
This log doesn’t set any targets. Your dry weight, fluid allowance, and BP range are still set by your care team, not by this printable — use the space at the top of the page to write down the numbers your team gives you.
